It’s cruel too of course and comedy can be cruel!, whatever, but it also relies on the false premise that someone with Tourette’s HAS control over what they say so the joke just isn’t insightful or original or good
It would be insane to not have some kind of joke about what happened but jokes about disabled people are so often just plain hacky, variations on “That’s weird!” “That’s annoying!” or in this case “Cut it out (or else???)!” It’s not as cruel as it is lazy writing
