I think that was because they were doing aggregate and Red Bulls won the second game and technically won the belt but Atlanta advanced on aggregate.

If you mean one team defending it throughout the playoffs it’s never happened. NE in ‘14 and TOR in ‘19 had it throughout but lost in the final. If you mean in general, it’s pretty common. 21 of 29 times it’s eventually won by the MLS Cup holder. Only time it was in the playoffs and didn’t was ‘18.
