Often times these are learned filler pauses that we insert like the "uh" or "um" you mention. You can become aware of this and work to eliminate it by slowing down and using silence.
I had this problem myself when I was young and when it was pointed out I worked to eliminate it as much as possible.

You can also just nicely point this out to them. They probably don't legitimately realize they're doing it, nor annoying people with it.
I find it interesting that whenever you see a member of Congress being legitimately investigated for the latest scandal or inappropriate actions the likely 100:1 odds are that it's a Republican. And you look at the news story and go "Oh, yep, indeed it is!"
