The spam is propagated through the free relays. You can filter those out and only read from the paid ones. But then you're also filtering out the people who don't pay for relay access.

You would have to relay it if you want others to read it. However, you can setup a private Nostr relay that only you and people can post to. The posts would be visible/public to anyone though.

So far the biggest pro Bluesky has over Nostr is the ability to delete posts but I am still struggling to understand the differences and the need for two competing services. My biggest Nostr apprehension is that if my private key gets compromised then everything is exposed. No way to change it.
