Listen, I suffer from painful side effects from HRT that nobody warned me about, I went through the medical system 3 times seeking treatment or even proper diagnosis. Talking to trans women saved me. You don't get "medical supervision" while going the government approved method.

If they can't pay the toll, this is another opportunity for adventure! Have the strongholder set the party a quest. The MU needs you to venture into a dungeon to steal something magic or a cleric needs you to deliver food or medicine to a village!

The lands surrounding a castle are usually controlled by the castle, and so there might be some cost to the party to pass through, whether it be winning that tournament or surrendering magic items to a MU or tithing to a cleric's church.

But don't just make them static! Castles have scouts and spies, and unless the party are sneaking through the lands, there's a chance they'll be contacted by riders from the castle. Chaotic strongholders will likely try to attack or capture the party, a neutral fighter might instead host a tourney!

First, decide who runs the castle; are they a magic user, a cleric or a fighter? Are they lawful, chaotic or neutral? Then decide who else lives in the stronghold chaotic clerics might have a retinue of ghosts, while a neutral fighter stronghold might be staffed with giants or gryphon riders.

A trap i often see new GMs falling into is thinking that they need to prepare absolutely every detail for every location before they can run a game! Castles and strongholds and their NPCs just need a few things that you can generate at random in order to create an exciting and memorable location!
