That's part of the DSM criteria itself, persistence in their gender identity. That doesn't require therapy sessions over a year or two to observe it directly, mind you, you can get that same information from records, interviews with parents/siblings/teachers/the child, etc.

The DSM-5 provides very clear guidelines for diagnosing gender dysphoria in children (the guidelines are less restrictive for adults). It's not intuition, it's the same way we diagnose any other disorder - you pick the diagnosis that fits the presenting symptoms the best.
