In my own (archivist) experience: I'd say the bigger problem than accidentally deaccessioning something valuable to an archives is not having the funding/storage to accept more future material that fits in your mandate.
Generally, a lot of thought and consideration goes into deaccessioning-
When we say "no, everything hasn't been digitized," I need you to understand that we really mean is that virtually nothing has been digitized. This is because the realm of primary sources that historians use is incomprehensibly large.
