Some of the unintuitive results are due to the pose. In case of your second example, woman with legs spread is almost always "sexually explicit", even if the naughty bits are covered.

As others already mentioned, it mostly helps accessibility, and has become something of a social norm here. Some people won't boost or otherwise interact with your posts if your images don't have alt text.
Alt text doesn't have to be overly verbose, just a few words on what the image is about.
