Highlights • We identified a pathway for airborne SARS-CoV-2 transmission between vertically adjacent hotel rooms. • Epidemiological and virus analysis suggested the two cases were linked despite no physical contact. • Aerosol tracing showed that the porous masonry exhaust shaft allowed vertical transfer between rooms. • Deploying a HEPA air purifier in the recipient room accelerated aerosol decay.
