NEW: Study finds little evidence supporting the safety & efficacy of Physician & Anaesthetic Associates in the NHS.
From @trishgreenhalgh.bsky.social & @martinmckee.bsky.social, the largest ever UK review raises critical questions about the rapid expansion of these roles.

People often talk about the time critical nature of sepsis. But it may surprise you to hear that the evidence to support βearlyβ antibiotics actually isn't a settled debate. Why is that? The key question is βearly in relation to whatβ? 𧡠1/n

Rant #5 Please don't tell me what the PaO2 is...
Less experienced critical care staff members are frequently exercised by arterial blood gas PaO2 readings.
What is the SpO2 I genially ask? In our target range? Well that's fine then.
Staff member shifting uneasily, "but the PaO2..."

NHS physician associates and doctors locked in 'toxic' staffing row
A toxic row has engulfed the NHS, say ministers. So why have doctors turned on physician associates?
This BBC piece is a very one-sided account of the issues in the use of PAs in the NHS www.bbc.co.uk/news/article...
It skims over the avoidable death of 30-year old Emily Chesterton, who died tragically after seeing the same Physician Associate twice.

NHS physician associates and doctors locked in 'toxic' staffing row
A toxic row has engulfed the NHS, say ministers. So why have doctors turned on physician associates?
This piece by BBC health correspondent Nick Triggle on the replacement of NHS doctors with PAs is disappointingly misleading through omission of key context & reliance on those busy, yet always conveniently anonymous, 'NHS senior sources'. 1/n

What is a common thread in medical tragedies?
Rescuers fail to cut through the βmind lockβ; they follow the primary caregiver down the wrong, fatal, path.
If we are going to train our way out of this problem, we need to train the rescuers.

One quote says it all: βQuick Reference Handbook sit on a shelf uselessly while a crisis is unfolding?β
I know that in emergency F-35 pilots handover aircraft to their ground teams, to go through a checklist. Before you shoot me down for an aviation reference - If they can we should be able too!

Sat listening at @TheBMA England #LMCConf where GP representatives meet on behalf of GPs looking after 69mn patients. We need a huge investment from the new govt. NHS gen practice is bedrock of NHS & only receives 6% of the NHS pie. Invest in us & we can deliver. GPs are incredible value for money

Huge news: Labour are looking to rewrite the NHS Long Term Workforce Plan according to Nursing Times.
There has been A LOT of criticism from the medical profession on the details of the plan, first drawn up by the Tories in 2023.
