I'd also get a foam roller and roll the shit out of your upper back. Use different arm positions and do it for about 2 minutes. Arms overhead, crossed both ways, out to the sides...
I think maybe the muscle that connects under your shoulder blade to the base of your skull could be contributing.

That's a nerve issue, but could hopefully be alleviated to some degree with some active release stretching, I think.
Tilt your head down, and then place your hand on the back of your head and press your head into your hand and hold for a 5 count, then release and re-stretch. Then repeat a couple x
