I would argue that a POCUS scan should have been performed well before the CT scan. A dysfunctional RV would have supported the need for contrast.
Plus I would have never started pressors and massive fluid administration without a glimpse of the LV.

Great thread, as always! It's never stressed enough: IVC is NOT a linear measure of loading status, just a very dynamic variable that you must fit in the whole context. It forces us to think, and that's a good thing.
