I’m seeing the doctor this week so I requested a more efficient means of accessing the form. They suggested fax. The third time I called the insurance company and asked them to fax over the form, it finally went through to the doctor’s office
The blank form has to come from the insurance company. No one at the insurance company seems to know if the form is available anywhere on their website. They “can’t” (won’t) email it to me or send it to me in thee secure messaging system. They offered to send it postal mail.
The hospital and insurance company are fighting so I have to get my doctor to fill out a form to ask the insurance company to cover my visit (this doc is at a major hospital that has been in network for the past 4 years that I’ve been seeing her)

ICE components are working with increasing volumes of criminal, civil, and regulatory, administrative documentation from numerous internal and external sources. The Government is assessing the marketplace for existing and emerging commercial off-the-shelf (COTS) solutions comparable to large providers of investigative data and legal/risk analytics. As well, the Government is seeking to understand the current state of Ad Tech compliant and location data services available to federal investigative and operational entities, considering regulatory constraints and privacy expectations of support investigations activities.

Me: "My dr. referred me to a specialist out of network. The plan says I'll owe 35% of the allowable amount if I see them. What's the allowable amount?"
Insurance: "We can't tell you that until after you've seen the doctor"
Me: "Why not?"
