En el cerebro, el MCT8 se expresa en los tanicitos humanos, las neuronas que expresan la hormona liberadora de tirotropina (TRH) y las células foliculosteladas de la hipófisis, y es probable que sea un regulador clave de la homeostasis de la TH en el eje hipotálamo-hipófisis-tiroides. La pérdida de la función del MCT8 da lugar a niveles elevados de triyodotironina (T3) circulante que suprimen la secreción de TRH por el hipotálamo y la secreción de hormona estimulante del tiroides (TSH) por la hipófisis. Esto va acompañado de una reducción de la secreción de tiroxina (T4) por la tiroides, debido tanto a la disminución de la secreción de TSH por la hipófisis, el efecto directo de la deficiencia de MCT8 en las membranas de los tirocitos, que dificulta la salida de TH, como al aumento de la conversión periférica de T4 en T3. Esto conduce a un aumento de la T4 y la T3 intratiroideas y a la perturbación del mecanismo de retroalimentación. En los riñones, el aumento de la actividad de la enzima yodotironina desyodasa tipo I (DIO1), responsable de la conversión de T4 en T3, va acompañado de una disminución de la excreción y del «atrapamiento» de T4. Se cree que esto da lugar a una rápida conversión de la T4 atrapada en T3 y a la liberación de T3 en la circulación por transportadores alternativos.
La deficiencia de #MCT8, o síndrome de #Allan-Herndon-Dudley, es un trastorno grave y raro del transporte de la hormona #tiroidea. En nuestro último artículo, revisamos los avances más recientes en fisiopatología, diagnóstico y tratamiento.
