A column in The Washington Post today cequates lifesaving gender-affirming care to lobotomies. This extreme comparison is not new, and in fact has been pushed by conservative media and politicians for years. π§΅
When I first covered this phenomenon in 2023, it was a frequent talking point on the right. So much so that lawmakers from TX to WI were adopting the same language. A Wyoming rep even proposed calling gender-affirming care "sexual lobotomy" at a federal hearing.

But being gay or trans is not a mental illness. Gender-affirming care is a means for trans people to pursue lifesaving treatment for gender dysphoria and a way to affirm their identities in everyday life.
In fact, lobotomies were once used as a means of attempting to convert or βcureβ LGBTQ people. The βFather of the Lobotomy,β Dr. Walter Jackson Freeman II, performed almost half of his nearly 3,500 surgeries on gay men to this end.

While patients under the age of 18 may access plastic surgery procedures under physician guidance and with the approval of a parent or guardian, this is atypical compared to the overall patient experience and the majority of cases within this data set are focused on the ages 18 to 19 years. Maturity β Adolescents typically experience changes in perception of body image, so it is important to assess the stability of each individualβs self-image before proceeding with plastic surgery. There are four attributes associated with body image that should be considered. These include physical reality of the appearance, perceptions of appearance, importance of appearance and the degree of satisfaction with appearance. In addition, adolescents may not have the physical and/or emotional maturity to choose plastic surgery. They may have unrealistic expectations about the surgery itself or about the outcome. They also may not understand that additional surgery may be necessary because of complications or a change in personal desire. Finally, they may not have reached full physical development. Informed Consent β It is important that the adolescent patient completely understand the procedure, possible complications and likelihood for additional procedures at some future date. As with all cosmetic procedures, appropriate informed consent will be required. The education process associated with an informed consent should help the patient and the parent/guardian understand the risks, benefits and potential complications associated with the procedure.
ASPS extends a lengthy caveat on how most of their numbers are from patients aged 18-19 (with no further clarity/delineation) and emphasizes the significance of parental consent & informed consent -- typical practice for trans health care for youth.

Apropos of everything, the American Society of Plastic Surgeons reports 2,774 breast augmentations and 1,169 breast lifts on patients 19 & under in 2024 alone. That's a combined 3,943 procedures in *one year* compared to the 3,678 gender procedures performed on patients 18 & under over *four years*
