My professional opinion as a pediatrician is you should not do this to kids.

Honestly we should pass a law requiring the time physicians spend on this be billable, and insurance companies have to pay it themselves without passing the cost onto the patient. Maybe that would incentivize them to stop making patients and physicians jump through these hoops to get essential care
And your experience also highlights, in addition to patients' stress, the challenges that prior auth causes for physicians who help us out when these challenges arise: they need dedicated support staff, and when they get dragged into "peer-to-peers" and protracted appeals, it's non-billable hours.

Every time I have to talk to an insurance company on the phone, I let the patient know. When they express frustration about it and ask what they can do I tell them to call their politicians and complain. It probably doesn't make a difference but at least they are aware

I've got two big problems with this.
The first is that it's dishonest.
Hospitals know how much revenue physicians bring in. Here, e.g., are figures from a 2016 survey.
Rather than "losing $300k", the average PCP/specialist *generated* revenue of $1.4 million and $1.6 million, respectively.
