In the past it was my custom to give up Twitter for Lent.
I gave up Twitter entirely a couple of years ago when it became muskified.
But this year I will be coming off BlueSky for Lent.

I know quite a few people in this country who can hardly speak any English. My grand-daughter and most of her peers, for starters.
Then again I didn't speak much English when I was 18 months old either.

Not unless you want to take the blame for every person in your social demographic.
Bad people exist in every group. That's not an excuse to use the actions of individuals to target those groups, particularly already marginalised groups

I can remember the days before thrombolysis. Read ISIS2 in Lancet as a Med Registrar and the next acute take gave my first shot of streptokinase - slow iv by hand as no infusion pump available. ECG looked awful, patient looked worse.

No offence taken, I know I am a dinosaur. I did old style gen med clinics with one boss when I was an SHO, but I also did Gastro, DM, endocrine and renal clinics as a junior.
But experience counts for a lot. By age 55 I had seen more cases of rare diseases than I had at age 35.

But this must be very frustrating for people unfortunate enough to suffer from rare conditions.

Of course, rare diseases often take longer to get to the bottom of than more common ones, but i don't think a new breed of GP would solve that.
More "ordinary" GPs, with more time, would however help.

BUT - many medical specialists do have a substantial element of Gen Med training, and will be aware of rare conditions which overlap with their specialism. So eg as a respiratory specialist i would be aware of things not directly lung related which might cause patient to feel breathlessness.

See this is where EM Drs have it all wrong. Instead of focusing on mundane stuff like initial management and saving lives, EDs should be improving flow by discharging ward patients to reduce bed occupancy.
