Medical systems are among the slowest to adopt new technology, so I'm sure those ridiculously long and repetitive questionnaires are a product of that. Not all doctors offices have started using electronic records and they hopefully at least use those forms to verify the data they do have.

We do 50% water changes with a water conditioner treatment roughly once a month with top-offs in between. It being a goldfish, the filter is roughly 5x the size for that tank, so I'm sure I'm accelerating evaporation with that as well.
I mostly marvel at what is possible when one DOES give something attention and care lol.
