Not sure. they do have the OMOP common data model where they try to standarize all the various hospital codes. Maybe the OHDSI book can help with answering your question? ohdsi.github.io/TheBookOfOhd...
They do have a very active community with regular calls. I just don't participate in them.
I'm not that connected with medical data as much these days, but this sounds like something the OHDSI folks have been working on (www.ohdsi.org/data-standar...).
I think Athena is their project of mapping all the vocabulary and ontologies that build on top of SNOMED: athena.ohdsi.org/search-terms...
