The report shows many deaths could have been prevented if successive governments had acted on long-standing calls to end womenβs imprisonment.

Safety in custody: quarterly update to June 2025
Quarterly update on deaths, self-harm and assaults in prison custody in England and Wales. Annual detailed data on assaults and self-harm in prison custody.
The latest Safety in Custody statistics show that in the 12 months to September 2025, there were 411 deaths in prison custody, an increase of 30% in the previous 12 months.
Of these, 96 deaths were self-inflicted, an increase from the 88 self-inflicted deaths in the previous 12 months.
