If partial afferents remain, could central targets (e.g., thalamus) be modulated through them?

Interested in how others conceptualize this. #pain

New case report published on trigeminal ganglion stimulation for refractory anesthesia dolorosa.

Rather than a binary β€œcomplete anesthesia + pain,” clinical nuance suggests a spectrum of deafferentation.

Thank you to @u2fp.bsky.social for the invitation to talk about the future of #neuromodulation for #spinalcordinjury
So many good speakers and great ideas

πŸ§ͺ Research Spotlight: Ziad Nahas, MD, MSCR, is studying how brain networks regulate mood in #BipolarDisorder & how instability drives shifts between depression & mania with PACE

πŸ‘‰ Goal: Test if targeting networks can improve mood & reveal mechanisms behind switches.

We are excited to be a part of this community to further efforts to understand and treat #bipolardisorder where we hope to bring biological insight back to psychiatry @umn-tc-aaup.bsky.social

electrodes can stay forever though like anything they can slowly degrade over time! We have some patients with electrodes in their brain for more than 20 years!

And it isn't just about invasive stimulation. It is a tool that we have amongst many and has its uses. But at the same time it sets the stage for validating noninvasive stimulation approaches. We can do invasive stimulation for those who truly need it. #depression

This is the future. The reality of biological #psychiatry. We listen to the patient, create a personalized map of their brain, use algorithms to disrupt stuck networks, and adapt the stimulation over time while continuing our quest to understand those networks.

What is really interesting is that these networks are embedded in the same networks that play a role in executive function and working memory as well as emotional regulation. @alexanderbherman.bsky.social

We have always known that there is the possibility of top-down modulation of pain circuits potentiating the endogenous analgesia network. The question has been how do we even think about trying to target. PFM seems to be a giant leap forward with @drdamienfair.bsky.social @ndosenbach.bsky.social

These patients and many others suffer from central neuropathic pain that often is due to #stroke or #multiplesclerosis or treatment of #trigeminalneuralgia resulting in #anesthesiadolorosa. These are some of the worst pain syndromes on earth!

β€œAn Action Networks Model for Pain”

We propose a new model for chronic pain β€” and highlight two functionally connected cortical networks that could revolutionize how we treat it.

Thrilled to drop this with @drdamienfair! Chronic neuropathic pain still disables millions despite meds & implants. We think the answer lies in action circuits

With a thousand people essentially choosing between slot machines, we were able to figure out that anxiety traits and apathy traits provide counterbalance pushing us to either search for better options or stick with what is working, guiding our decisions #universityofminnesota
