If I had to guess why it happened: Watt & Volt didn't have the ability to grab & pull boxes until later in the development cycle, they could only push them by walking against them. So when I placed the box there, I added a ramp on the right to prevent the box getting stuck against the wall...
I just checked and... you're totally right. There are 2 unreachable tiles under that ramp, as you can see in the attached image with the ramp removed. I'm so sorry! π
