What Is Reflective Functioning? Mentalization, Attachment Theory & RF Scoring with Dr. Miriam Steele
In this episode, Dr. Puder hosts a conversation with Dr. Miriam Steele, a leading expert in reflective functioning (RF), mentalization, and attachment theory. They explore the origins of RF from the pioneering work of Peter Fonagy and John Bowlby in the London Parent-Child Project, its role in predicting secure attachments and sensitive parenting, and distinctions from empathy. Conversation topics include cutting-edge research on mentalization-based treatment (MBT) and transference-focused psychotherapy (TFP) for borderline personality disorder (BPD) and eating disorders, therapist RF's impact on patient outcomes, body image representations, and smartphone effects on parent-child bonds.

What Is Restless Legs Syndrome?
When a patient is seen with restless legs, most assume that it's simply due to bursts of hyperactivity or agitation. However, if this restlessness is associated with unpleasant, uncomfortable sensation and is worse at rest, it is most likely Restless Legs Syndrome (RLS).
In this short video, Dr Sanil Rege, a psychiatrist, explores the defining features of RLS and the probable behavioural profile that emerges when it overlaps with ADHD.
Core Concepts Discussed:
What Is RLS?: Recognising RLS as an unpleasant, uncomfortable sensation in the legs where some relief is obtained with moving the legs.
Risk of Misdiagnosis: Noting that RLS can be often misdiagnosed as medication-induced akathisia or generalised agitation.
Timing as Key Differentiator: Identifying timing as a key differentiator between RLS and akathisia, as RLS tends to occur towards the latter half of the day as the person is trying to go to bed.
RLS and ADHD: Outlining the probable behavioural profile when RLS and ADHD co-occur, including worsening attention across the day, daytime fatigue, and night-time activation.

My depression felt creatively expansive. Now Iβve overcome it, how do I keep the meaningful parts? | Leading questions
Darkness can present itself as truth, advice columnist Eleanor Gordon-Smith writes. But love, joy and awe are far richer experiences
Iβve overcome a depression that was both debilitating and strangely vivid and creatively expansive. How do I step back into ordinary life without losing the parts that felt meaningful and alive?
During that time, I immersed myself in creative work, I was writing poetry, painting my feelings, and reading books that echoed my inner world, and I found a depth that felt honest, vivid and alive. I gravitated towards art and film that explored lifeβs darker edges and through them I gained a perspective that felt richer and more truthful than everyday chatter. Those pursuits became my comfort and my way of making sense of things.

Creativity, Mania, and Finding Balance on World Bipolar Day
One writer reflects on bipolar disorder, Van Gogh, and the hard-won stability that makes creativity feel safer. By the time I was 16 years old, Iβd convinced myself that I was a creative genius. I knew nothing yet of bipolar disorder, but the budding manias of my teenage years had flowered into an obsessive pursuit...

Before You Try Viibryd (Vilazodone), Watch This
In this video, I walk through how Viibryd actually works, why itβs different from standard SSRIs, and what that difference means for sexual side effects, emotional flattening, anxiety, and tolerability. Weβll talk about the real trade-offs, the GI and food rule most people arenβt warned about, and how Viibryd compares to common alternatives like bupropion and vortioxetine.
Iβm Dr. Sulman Aziz Mirza, a psychiatrist triple board certified in adult psychiatry, child and adolescent psychiatry, and addiction medicine. My goal is to help you understand psychiatric medications in a way that feels human, honest, and grounded in real clinical experience β not marketing.
If youβve tried an antidepressant before and are deciding what to do next, let me know in the comments what trade-offs matter most to you. And if you want a deeper dive into vortioxetine (Trintellix) and how it compares head-to-head with Viibryd, check out the next video linked here.
This video covers: Viibryd review, vilazodone side effects, antidepressants and sex drive, SSRI sexual side effects, alternatives to SSRIs, and how to choose the right antidepressant after the first one didnβt work.
This video is about Viibryd and mental health and psychiatry, as part of our series on mental health and psychiatry. Subscribe to our channel if you want to learn more about mental health and psychiatry!

Viibryd (vilazodone) is often described as an antidepressant thatβs easier on sexual function. But that claim deserves nuance β especially if youβre considering it after another antidepressant didnβt work the way you hoped.

How to Overcome Guilt: Break Free from Unreasonable Expectations with Jennifer Reid, MD
In this episode, we welcome psychiatrists Dr. Jennifer Reid (author of Guilt Free: Reclaiming Your Life from Unreasonable Expectations) who explores why guilt feels so overwhelming amid post-COVID pressures, perfectionism, and endless roles. Dr. Reid and Dr. Puder differentiate guilt from shame, highlighting adaptive healthy guilt for repairing relationships versus maladaptive generalized guilt tied to depression, anxiety, burnout, and cognitive distortions. The discussion covers childhood roots like parentification and socialization, narcissism's reduced guilt, therapist/doctor identity struggles with moral injury, and practical strategies like boundary-setting, reframing expectations, and safely referring unsafe patients without guilt. Ideal for anyone battling unreasonable self-expectations, mental health providers seeking tools to manage guilt in patients and themselves, or listeners wanting to break free and reclaim emotional freedom through self-compassion and realistic accountability.

Alex Lee on using improv and comedy to bring history to life
Comedian, writer and host Alex Lee joins Osher to talk about her new ABC panel show Tonight At The Museum β and how she's using comedy to help us make sense of history, identity, and the stories we choose to preserve.
From the chaos of writersβ rooms and the power of improv, to filming inside a working museum in Tasmania, Alex shares how the show was built from the ground up β and why museums are no longer just dusty rooms full of old stuff, but living reflections of who we are.
We explore:
π Why improv comedy is a secret superpower in TV (and life)
𦴠The strange, funny and confronting history behind museum collections
π A jaw-dropping extinction story that will stay with you
π² Dungeons & Dragons, creative identity, and designing the person you want to be
π¬ What itβs like having pyrotechnics on standby while you nail a blockbuster film line at 4am (while very pregnant)
π How museums are reckoning with colonial history β and why humour helps us have harder conversations

Gene Test Trap: Why Pristiq May Misguide Your Meds! #shorts
Gene tests promise personalized medicine, but the βgene test trapβ can quietly misguide your treatment, especially with medications like Pristiq. Many tests focus on how fast your body metabolizes drugs, not whether the drug is the right match for your symptoms, biology, or side effects. This can create false confidence and lead to meds that look βcompatibleβ on paper but fail in real life. In this video, we break down how pharmacogenetic testing works, where it helps, where it falls short, and why real-world response, lifestyle, and clinical judgment still matter. Smarter choices start with better questions, not blind trust in test results. Gene Test Trap: Why Pristiq May Misguide Your Meds! #shorts
Pristiq (desvenlafaxine) often appears "clean" on gene tests, but real-world results are mixed. Don't let a report box you inβfocus on the person in front of you, not just the paper.
