Patients Vs. Profits A veteran psychiatrist shares his view of how private health insurance companies dictate — and limit — ADHD care in America.

"The only way to perpetually increase the profits demanded by shareholders is to deny legitimate claims or otherwise create obstacles that prevent or delay payments to providers." - Dr. William Dodson

Yesterday, revenge sleep procrastination came up with half my clients.

I’m at the end of a school vacation week and two snow days making things busier than usual. So, I don’t want to do anything.

I’ve get a new term: revenge responsibility procrastination.

But, I’m still off to edit a podcast.

"Post-COVID-19 cognitive dysfunction appears to follow a
temporal course, evolving from acute focal deficits in attention,
working memory, and executive function to more persistent
multidomain impairments..
This analysis shows that several inflammatory and viral cytokines
remain elevated beyond the acute phase and are associated with
cognitive deficits..
The prevalence estimates [of cognitive dysfunction] vary
depending on the methodology used and the time of assessment
but generally range between 58% and 81% in hospitalized samples,
reflecting a significant impact on cognitive function.
In addition, a substantial number of patients, approximately 34%,
report subjective complaints, mainly related to attention and
naming..
Patients who experienced acute neurological symptoms such as
headache, anosmia, or dysgeusia were more likely to show deficits
in domains such as attention, memory, and executive functions.."

I hear you. It is painful. And unsettling!

I'm just saying that we so often talk about it like it's this unique thing when in fact it's a form of anxiety and that can help us address it more effectively.

psst...
Rejection sensitivity isn't some mysterious, unknowable thing. It's just a form of anxiety.
So anxiety management techniques will help.
Pass it on.

Most of the time when you clean a room, the mess gets worse before it gets better.

This post is both literal and a metaphor. Use it as you see fit.

Thoughts on ADHD & Goals for 2026 It’s January 10th.  By now, odds are you’ve already fallen down on some of your resolutions.  To that end, I want to talk about two of my goals that I can almost guarantee were more delayed than yours...

In which I discuss taking 16 years to accomplish not one, but two! five-year goals.

And share three takeaways from the experience.

I hear you. I was there nit that long ago, as you know. (And got hit with a stomach thing on Monday.) I'll try to be as light as lift as I can for you. Seems like we're gonna coast downhill for a bit anyway.

There isn’t a secret bonus final episode of Stranger Things, and you don’t hate the ending as much as you think. You’re just sad the show is over. And that’s okay. You’re allowed.

Idea to stop doomscrolling:
Take a picture of what you'd rather do than doomscrolling - the book you want to read, the place you'd rather walk, whatever - then make that picture the lock screen on your phone.
