After the cutscene where Ralsei tries to help the Old Man reach his book, Ralsei gains the title of "Stool Boy" which only lasts for this one room. And every time you backtrack to this room the title returns.

In the ruins there was a secret room with a Goner who said something cryptic and Gasters (multiple) would randomly appear and disappear on screen
