I started reading the opinion and got the gist but then couldnt wait and skipped to the dissent. TLDR: The stupid ones dissented.

My small company ran with a self-insured plan when it was available some 15 years ago or thereabouts. We saved enough money to cover 100% instead of 80% of employee premiums the next year.
This left money in our hands instead of the insurer, so they ended offering such plans to small biz.
