And I think there’s maybe a tautological thing where, if we understood MCAS itself at a deeper level, maybe we’d have better treatments, but then it probably also wouldn’t be “MCAS” but would get a better name to reflect actual pathology instead of being “syndromized”

At least as far as I understand the science today (so, a flawed view!) it’s not necessarily “the one pathway by which MCAS develops” as much as “inflammation, autoimmunity, and gut dysbiosis eventually flip the MCs into hyperreactivity”, which is just the old problem that every case is different

I think the trickiness is there isn’t really anything to do, other than “try to aggressively treat ME/CFS and associated conditions, and maybe preemptively treat MCAS (i.e. antihistamines, MC stabilizers, perhaps mild diet changes)”

(admittedly this is partly a linguistic concern — dhyāna/jhāna and bhāvāna both get translated into English as “meditation” despite referring to drastically different things, and finding material that focuses on the former is much more difficult)
