In Super Mario Sunshine, each plank of the Pianta Village bridge has separate collision for an unknown reason. This causes microscopic invisible gaps to form between the planks that extremely unfortunate players can slip through and die. Below is a compilation of such cases.

Actually, its his visors. It "erase" parts of his memories, so he can have better focus on his missions as an agent. He wasn't forced to wear them, he chose to wear them.

No death, people considered Noskage to have mysteriously gone missing.

Unfortunately, his visors wasn't properly prepare for his Mega form, & his power would be too much for the visor to withstand.
The more critical hits 96 lands, the more damage his visor takes, bit by bit.
