A figure from a scientific paper about autophagy, which is quite hierarchical. Box A is much bigger and more prominent, with a thicker border, bolder colors, and bigger text. This makes it clear that this box is more important in the hierarchy. Within box A, a flow with different steps is visible.
Visuals with a clear flow can get away with a less hierarchical layout. And very hierarchical visuals don't need a super obvious flow. They can live together - this visual has both, for example - but are more of less complementary.
