CT chest shows a markedly dilated, fluid-filled oesophagus with smooth distal tapering—classic achalasia. Diagnosis confirmed on high-resolution manometry (Type II). Post-POEM, dysphagia improved from 3 to 1, showing excellent multimodal concordance.
